I, for myself, my child/children or ward/wards sign this Waiver and Assumption of Risk in consideration of the opportunity to use the facility, or to participate in any activities, events or parties at KIDZ FUNTOWN/PHAMILY FUN & FITNESS.I, for myself, my child/children or ward/wards acknowledge and understand that there are dangers and risks associated with the activities at/by KIDZ FUNTOWN/PHAMILY FUN & FITNESS and agree to assume all risk of personal injury, including the potential for paralysis and death.I, for myself, my child/children or ward/wards agree to follow the safety instructions provided and acknowledge that failure to do so may result in expulsion or immediate removal from KIDZ FUNTOWN/PHAMILY FUN & FITNESS.I, for myself, my child/children or ward/wards and on behalf of myself or their heirs, assigns, personal representatives and next of kin, HEREBY HOLD HARMLESS KIDZ FUNTOWN/PHAMILY FUN & FITNESS, its owners, members, employees, and equipment manufacturers from all liability for any such personal injury, disability, death, damage to person or personal property to the fullest extent of the law.I, for myself, my child/children or ward/wards fully understands by signing this waiver agrees everything stated above and will grant admittance into KIDZ FUNTOWN/PHAMILY FUN & FITNESS and will be stored on file for one year.Photo ReleaseFor valuable consideration received, I grant to KIDZ FUNTOWN/PHAMILY FUN & FITNESS and their legal representatives and assigns, the irrevocable and unrestricted right to use and publish photographs of myself, my child/children or ward/wards, or in which I, my child/children or ward/wards, may be included, for editorial, trade, advertising, and any other purpose and in any manner and medium; and to alter and composite the same without restriction and without my inspection or approval. I hereby release Photographer and their legal representatives and assigns from all claims and liability relating to said photographs.
